Hyperhidrosis is not rare. According to the 

, approximately 385 million people worldwide live with hyperhidrosis.

In fact, they note that the amount of people living with chronic, excessive sweating is even higher than the amount of people living with other dermatologic conditions like psoriasis.

Just how common is hyperhidrosis in real-world comparisons?

The prevalence of hyperhidrosis is much higher in younger populations than in the general population, with 8.8% of individuals between the ages of 18 to 39 years old living with the condition.

Ergonomics in dermatological surgery: An international survey among dermatologists

A global survey of dermatological surgeons found a high prevalence of musculoskeletal pain, particularly in the neck, shoulders, and upper and lower back. Despite widespread awareness of poor posture during procedures, few surgeons actively monitored their positioning. Researchers recommended behavioral strategies, such as alternating between sitting and standing and taking frequent breaks, to alleviate discomfort. Additionally, workstation modifications—including surgical loupes, proper lighting, and adjustable stools—were identified as essential for minimizing strain and improving long-term musculoskeletal health.

Effectiveness and tolerability of an emollient "plus" compared to urea 10% in patients with mild-to-moderate atopic dermatitis

A clinical trial comparing a modern emollient "plus" moisturizer containing bacterial lysate to a standard urea 10% moisturizer for mild-to-moderate atopic dermatitis (AD) found the emollient "plus" to be more effective in improving skin barrier function and reducing symptoms. Sixty participants applied their assigned moisturizer twice daily for 12 weeks, with assessments at multiple intervals. The emollient "plus" showed greater improvements in transepidermal water loss, skin pH, SCORAD, and hydration, with significant differences in eczema severity, itch, and quality of life scores by week 12. Both treatments were well tolerated, but the findings support the emollient "plus" as a superior option for AD management.

Improving cardiovascular outcomes in the psoriasis cohort. Psoriasis and cardiovascular disease-clinician knowledge, practice and perceptions

A cross-sectional study assessed clinician and patient knowledge, perceptions, and practices regarding the link between psoriasis and cardiovascular disease (CVD). A survey of 298 clinicians and 102 patients revealed that while most dermatologists and rheumatologists recognized the association, awareness was significantly lower among cardiologists and general practitioners. Only a small percentage of patients were aware of the connection. Clinicians favored primary care physicians for CVD risk management, whereas patients preferred specialist or multidisciplinary care.

Rationales of cold plasma jet therapy in skin cancer

A review of medical gas plasma therapy in skin cancer treatment highlighted its potential as a complementary approach for basal cell carcinoma, cutaneous squamous cell carcinoma, melanoma, and actinic keratosis. The study discussed the mechanisms of gas plasma, which generates reactive oxygen and nitrogen species, and its established safety in dermatology, particularly for nonhealing wounds. It also explored preclinical and early clinical evidence supporting its use in dermato-oncology and outlined strategies for integrating gas plasma into existing skin cancer treatment protocols, encouraging further clinical research to advance its role in cancer care.

International consensus statement on the use of ultrasound in hidradenitis suppurativa

A review of ultrasound (US) use in hidradenitis suppurativa (HS) highlighted its value in diagnosis, scoring, and disease assessment, despite the absence of international consensus. To address this gap, a Delphi study gathered 24 experts across 14 countries to establish recommendations on US applications in HS. The panel reached strong consensus on key aspects, including diagnostic criteria, staging, monitoring, surgical planning, and research integration. While challenges remain in implementation, the task force strongly advocates for US as a crucial tool in HS management and clinical practice.

wants to hear from you. What treatment options do you most commonly rely on when it comes to managing or treating hyperhidrosis in your patients?

 Antiperspirants containing aluminum chloride, like Drysol and Xerac AC, are often the first line of defense for those dealing with hyperhidrosis. When applied to dry skin before bedtime, they work by blocking sweat ducts and reducing excessive sweating.

 For localized hyperhidrosis affecting the face and head, prescription creams containing glycopyrrolate or wipes soaked in glycopyrronium tosylate (Qbrexza) can provide relief. These topical solutions are effective in reducing excessive sweating in the affected areas.

 Certain oral medications can be used to block the nerves that trigger sweat glands, reducing sweating in some individuals.

 Some medications typically used to treat depression may also help decrease sweating, particularly in cases where anxiety plays a role in exacerbating hyperhidrosis symptoms.

 Botox injections have gained popularity as a highly effective treatment for hyperhidrosis. By blocking the nerves responsible for activating sweat glands, Botox can provide relief for several months. 

Surgical and Other Procedures for Hyperhidrosis

 Iontophoresis is a home treatment method that involves soaking the affected areas, such as the hands or feet, in water while a device passes a mild electric current. This current blocks the nerves responsible for sweating. The treatment can be performed at home with a prescribed device.

 Microwave energy is used to destroy sweat glands in the armpits. The procedure typically requires two sessions, each lasting 20 to 30 minutes, and spaced three months apart.

 For individuals with localized hyperhidrosis in the armpits, sweat gland removal may be considered. This can be achieved through various methods, including scraping, suction, or a combination of both.

Hyperhidrosis management involves a range of treatment options, from topical and oral medications to surgical procedures. Prescription antiperspirants and creams are commonly used for localized sweating, while nerve-blocking medications and antidepressants can address systemic symptoms. Botulinum toxin injections offer temporary relief by inhibiting nerve signals to sweat glands. Surgical interventions, such as iontophoresis, microwave therapy, and sweat gland removal, provide more permanent solutions for severe cases. These treatments cater to varying degrees of hyperhidrosis severity and patient needs, offering both temporary and long-term relief options.

Gooderham continues a discussion on new data reinforcing the safety of topical delgocitinib, with minimal systemic absorption even under maximal use conditions.

 in Orlando, Florida, Melinda Gooderham, MD, MSc, FRCPC, a dermatologist and clinical researcher from Peterborough, Ontario, Canada, provided further insights into the safety and systemic exposure profile of delgocitinib cream (LEO Pharma) for chronic hand eczema (CHE).

, Gooderham emphasized the importance of safety data, global regulatory approval considerations, and the potential for expanded indications and future comparative studies. Readers can view part 1 of this discussion 

Gooderham reiterated that delgocitinib’s systemic absorption remained minimal even under maximal use conditions. This conclusion builds upon previous research that assessed systemic exposure in both the CHE patient population and a phase 1 trial evaluating delgocitinib in moderate to severe atopic dermatitis.

"Because we had previously reported on the minimal absorption in the past in the whole cohort, looking at those with the maximal use, there was nothing unexpected," Gooderham explained. "It kind of confirmed what we thought—that even patients using a higher amount of product or covering more body surface area in the phase 1 trial for atopic dermatitis—it wasn't surprising; it was more reassuring."

The findings reinforce the safety profile of topical delgocitinib and provide reassurance to both prescribers and patients. Notably, even in cases where patients use the product on larger areas, there was no indication of concerning systemic absorption.

As delgocitinib has already gained approval in some regions, Gooderham noted that additional regulatory submissions and approvals worldwide will benefit from this safety data. 

"Moving forward, delgo[citinib] is already approved in a couple of regions, but there will be regulatory submissions and approvals more globally, and having this safety data will help with those approvals," she said.

Furthermore, these findings could influence product labeling by offering more comprehensive safety information to prescribing physicians. 

"Maybe it will also impact the label that is put on these products," Gooderham added, highlighting the potential for expanded indications or more detailed guidance on usage.

While delgocitinib’s safety and efficacy profile is promising, Gooderham underscored the importance of future comparative studies. 

"As far as the development, hopefully we could have in the future some head-to-head studies," she said, "so we can look more at how it's working compared to a topical steroid or a topical calcineurin inhibitor or PDE4 inhibitor. Head-to-head studies are always great for informing prescribers and patients when they're making choices."

Additionally, Gooderham emphasized the necessity of real-world data to assess delgocitinib’s long-term effectiveness and safety in clinical practice. 

"I think we'll also need to have some real-world studies come out on how delgocitinib is working in the real world as far as efficacy and safety, of course," she noted. "That’s what we're trying to show in this poster—that there's less concern of safety issues when you're using a JAK inhibitor topically."

Despite delgocitinib’s current approvals for CHE, Gooderham acknowledged that clinicians might prescribe it for other types of eczema. 

"One thing that I didn’t mention earlier is that although delgocitinib is approved for chronic hand eczema, we know that patients may use it on other areas of eczema, or prescribers might be using this product off-label," she said.

This is particularly relevant given the reassuring findings on systemic absorption. "Having this data, knowing that the absorption is negligible, even under maximal use conditions, I think moving forward, will open up options for patients and potential future indications," she added.

At the 2025 AAD Annual Meeting, Gooderham shared data on the negligible systemic exposure of delgocitinib cream for chronic hand eczema.

Melinda Gooderham, MD, MSc, FACPC: Minimal Systemic Exposure of Delgocitinib Cream in Chronic Hand Eczema

 in Orlando, Florida, Melinda Gooderham, MD, MSc, FRCPC, a dermatologist and clinical researcher from Peterborough, Ontario, Canada, presented findings on the systemic exposure of delgocitinib cream (LEO Pharma) under maximal use conditions in chronic hand eczema (CHE).

The study, presented in a poster titled, "Delgocitinib cream has negligible systemic exposure in patients with maximal use of delgocitinib cream for the treatment of chronic hand eczema," was aimed at evaluating the systemic exposure of twice-daily delgocitinib cream in patients with CHE using it at the upper quartile of usage during the phase 3 DELTA 2 trial.

In addition, the study compared the systemic exposure of delgocitinib cream with data from phase 1 trials, which investigated the use of delgocitinib in moderate to severe atopic dermatitis. The goal was to analyze systemic absorption differences between topical application on a larger body surface area and oral administration.

 spoke with Gooderham to discuss highlights and takeaways from the research.

"We have previously presented on the systemic exposure, but the purpose of this poster at the AAD was to look at the systemic exposure of twice daily delgocitinib cream under maximal use conditions," Gooderham said. "And then we wanted to compare that systemic exposure with data from the phase 1 trials which looked at delgocitinib cream used for moderate to severe atopic dermatitis, so on a wider body surface area, as well as looking at an oral dose that was given in a phase 1 trial, just to compare the systemic absorption of topical use on a larger body surface area, also one that was provided orally."

The study demonstrated minimal systemic absorption of delgocitinib cream, with the highest observed peak systemic exposure recorded at 7.2 ng/mL. In comparison, an oral dose of delgocitinib (1.5 mg) administered in a phase 1 trial resulted in a much higher mean exposure of 99.3 ng/mL. 

Gooderham highlighted the clinical significance of these findings: "I think the most important clinical implication is the lack of side effects. We know that JAK inhibitors have been given a black box warning, which makes both prescribers concerned; it makes patients concerned. It showed that there's negligible systemic exposure, which provides a lower risk of systemic side effects. It gives it a more favorable safety profile, and patients will be more comfortable using this on a longer term basis."

The safety profile of delgocitinib cream was favorable, with adverse events reported in 45.7% of patients in the delgocitinib group, and 44.7% in the vehicle group. The most common adverse event was COVID-19 infection, with only one serious adverse event (1.1%) in the delgocitinib group, which was unrelated to the treatment. Importantly, there were no reports of malignancy, major cardiovascular events, or venous thromboembolic events.

Gooderham noted, "We don't have a lot of maximum exposure done with topical steroids that we're commonly using on large body surface areas for our patients. It's nice to be able to reassure patients that these studies have been done when they're putting these creams on for chronic hand eczema."

Gooderham also placed delgocitinib in the broader context of topical treatments, comparing it to other options such as topical corticosteroids and topical calcineurin inhibitors. She added, "We know there's other systemic side effects or local side effects with the use of topical corticosteroids. There are issues with using topical calcineurin inhibitors, but recent data looking at topical JAK inhibitors, we have another with topical ruxolitinib, which also showed limited systemic absorption with topical use. So I think this is adding to that safety profile with these medications so that they can be used with comfort and prescribed by prescribers with comfort."
