Hidradenitis Suppurativa: Practical Tips and Considerations Regarding Diagnosis, Treatment, and Patient Communication

Hi, and welcome to this Dermatology Times Partner Perspective video series. I’m Tiffany Mayo, associate professor, vice chair of diversity and inclusion, and director of the clinical research unit at the Heersink School of Medicine department of dermatology at the University of Alabama at Birmingham. In the first video, we discussed the clinical features, patient burden, and challenges of diagnosis of hidradenitis suppurativa, or HS. In this final video, I’ll wrap up the conversation with a few final thoughts.

This episode, "Hidradenitis Suppurativa: Practical Tips and Considerations Regarding Diagnosis, Treatment, and Patient Communication" is sponsored by Novartis, and I have been compensated for my time.

I mentioned in the first video that the average time of delay in HS diagnosis is 7 to 10 years.

 During this delay, patients see an average of 3 different providers and receive 3 misdiagnoses before receiving the correct diagnosis, which is typically given by a dermatologist.

 These are general statistics, but I can share here that most of my patients report similar real-world experiences. From the patient perspective, this delay causes frustration and distrust. From a disease management perspective, a delayed diagnosis can lead to greater difficulty in resolving HS symptoms and progression.

Upon diagnosis, it is important to name the condition and help the patient understand what it is and how and why it needs to be treated. Patients with HS frequently have questions that can culminate in a time-intensive visit, involving at least 30 minutes of extensive counseling and additional time for prior authorization of medications.

 It’s understandable that recurrent visits of this type may not be practical for all HCPs; however, we are all responsible for ensuring that patients are accurately diagnosed and are on the right track for optimal disease management. Collaborating with local HS experts or other physicians who can help manage the disease is key when this is the case. Because there can be a delay in access to these physicians, it is recommended to start patients on an initial regimen but inform them that they will require follow-up and, likely, treatment escalation that will be handled by a colleague you are able to name.

Resources are also available to help with patient education and disease management. Foundations such as the US and Canadian HS Foundations play an important role in educating patients and clinicians through symposiums and other efforts.

 There are many tools HCPs can find helpful, including prior authorization templates, treatment guidelines, and HS specialty clinic directories.

I would like to summarize this video series by reiterating HS is a chronic, painful disease with a substantial impact on patient quality of life. There is typically a delay in time to accurate diagnosis and the disease is often suboptimally managed. Though more work is needed, we can support our patients by helping them understand the disease and by appropriately managing it. Thank you to our viewers for watching.

These grants aim to support researchers at different career stages, from early investigators to established practitioners, by providing funding to develop new treatment insights, improve patient care, and stimulate innovative, translational research. Below is an overview of the available grants:

 This grant seeks to promote clinical and translational research projects that can meaningfully improve patient care in the field of HS. Researchers are invited to submit applications that propose new approaches to understanding HS, including studies on disease mechanisms, therapeutic strategies, or patient-centered care models. Award amounts range from $5,000 to $20,000, allowing researchers flexibility to design studies that fit their project scope. A minimum of five grants may be awarded annually, subject to funding availability. This grant is ideal for early-stage investigators, as priority is given to applicants who are new to HS research. 

 Aimed at encouraging novel research capable of attracting long-term funding from the National Institutes of Health (NIH) or similar agencies, this grant supports innovative approaches in HS research. Eligible researchers can apply for up to $75,000 to fund one-year projects. The goal is to bridge the gap between basic science and clinical practice, enabling researchers to conduct studies that have the potential to translate directly into improved patient outcomes. Applications are welcome from early-stage investigators and collaborative research teams within an institution. 

 This grant supports trainees and early-career faculty interested in gaining hands-on experience in managing HS under the guidance of an established HS expert. The award provides up to $4,000 to fund up to four weeks of mentored clinical practice and procedural training. This unique opportunity aims to nurture the next generation of HS specialists by creating pathways to learn from seasoned practitioners and enhance skills in clinical care, research, and patient education. Priority is given to applicants from areas without access to an HS clinic.

All applicants will be notified of acceptance at the American Academy of Dermatology Annual Meeting in March 2025.

Hyperhidrosis is not rare. According to the 

, approximately 385 million people worldwide live with hyperhidrosis.

In fact, they note that the amount of people living with chronic, excessive sweating is even higher than the amount of people living with other dermatologic conditions like psoriasis.

Just how common is hyperhidrosis in real-world comparisons?

The prevalence of hyperhidrosis is much higher in younger populations than in the general population, with 8.8% of individuals between the ages of 18 to 39 years old living with the condition. 

When comparing these statistics to everyday, common occurrences like hair color, the number of people with hyperhidrosis is greater than the 1% of individuals with natural red hair and 2% of individuals with natural blond hair. 

Despite how common hyperhidrosis is, it is estimated that 27% of those with the condition never receive a diagnosis.

 However, symptoms of psoriasis can start at any age and stage of life.

The majority of patients with psoriasis will experience their first flare when they are between the ages of 15 and 35. Despite this, nearly 1 in 3 patients are under the age of 20 at the disease's first onset.

"While it can begin at any age, psoriasis has 2 peaks of onset, the first at age 20 to 30 years and the second at age 50 to 60 years," according to April Armstrong, MD, MPH.

 "It affects men and women equally but is more common in non-Hispanic whites. Some patients are more prone to developing psoriasis, especially if there is a family member with psoriasis."

Dermatology physician associates took a break from learning and networking to give back to the community during the Society of Dermatology Physician Associates (SDPA) 2024 Fall Dermatology Conference in Las Vegas, Nevada, by volunteering with the Sun Bus.

PA clinician-volunteers at the Sun Bus, which was parked outside of the conference hotel, screening individuals for skin cancer and sun damage as well as educating interested individuals about sun dangers and sun protection in conjunction with the Sun Bus. The event was organized by the philanthropic arm of the SDPA, 

. DPAF hosted a similar sun cancer screening with the Sun Bus during the 2024 Society of Dermatology Physician Associates Summer Conference in San Diego, California, in June.

Established in 2015 to help dermatology PAs expand their leadership and contributions to the field of dermatology, the mission of the Dermatology PA Foundation (DPAF) is to “inspire and invest in Derm PAs to succeed in professional development and philanthropic service to impact communities and provide inclusion for all.” Faseeha Raza, MMS, PA-C, the current chair of the organization, is proud of its commitment to give back to the community and has encouraged the sense of volunteerism.

The Sun Bus, which was acquired in May 2019, comes courtesy of the Colorado Melanoma Foundation. Its goal is to deliver free skin checks and promote sun safety and skin cancer awareness.
